In the Settings page of a DIY form, allow the user to choose the Contact Method rather than having just a system default.

To be able to identify and report by journal contact method and include contacts added via DIY forms.

      The contact method defaults to email in order to record the email confirmation that is sent to the constituent due to the form being submitted.  You can utilize hidden user defined fields under the Settings area of a DIY form if you'd like to track and report on submissions.  Please reach out to Support if you need assistance in setting this up.
