Add an optional donation field to contact pages

It could be used for event registration where you are asking for a suggested donation that is not required for attendance, as well as another avenue to gently ask (but not require) a donation while growing your database.

    At this time we do not have the option to have a "combo" page that allows donations and/or just contact info.  At this time we have no plans to add this functionality.
