Please populate the title field in 'Documents' embedded 'Contact us' template

Please can you populate the Title field when creating a new embedded form 'Contact us'. Title is a drop down field that hasn't been populated with [Mr, Mrs, Miss, Ms, Dr etc.....]  and on preview mode you don't get to select or type your own title.  HELP.

    After some testing, this appears to be working as requested.  You may need to re-publish your page for this to take effect.  If you are still having issues, please reach out to our support department.
