Be able to select name of DIY form as part of criteria for query

This way we could query on the transactions that come through a specific form instead of having to use a UDF, Fund or campaign

Add DIY Page Name to Query Merged

it would be helpful to have the ability to include the DIY Page Name in a query when multiple fundraisers have the same Fund. This way we can pull a report based on the DIY Page Name.
