We would like to create a 'How are we doing' Survey to send out to segmented donors.  We would like to know their thoughts on what we are doing well and how we can improve.  This survey would be something they could complete electronically and then they could email it back.  It would be even better if we could create fields that could capture that data so we could run reports on the 'How are we doing' results.
