Add field on DIY donation page asking donors to agree (or not) to cover credit card fees

This would allow donors to agree to cover the credit card fees associated with their donation; thereby, allowing the full donation amount to go directly to program expenses. Fees have become a large expense for our non-profit, thus causing us to research which fundraising programs offer this option.
