Allow DIY form to append phone or e-mail if not already present on account

When a donor provides e-mail and phone (or any information) on a DIY form, the database  looks to see only if an account already exists. If it exists, it ignores all the other data like the phone and e-mail even if those fields are blank on the account. Which means the donor entered two REQUIRED DIY fields, but that data is not on their account unless the user figures this out and adds it manually.
