Allow payment information to be customizable.

We are looking for ways to save on credit card fees. We were wanting to have EFT be the default for payment type on the DIY forms with credit card still being an option.

    This is extremely important for us. We would like to set this for the monthly donor signup form but still have the credit card option for the one off donor.
