Add options for voice and mobile phone number entries to DIY forms

Currently there are limited options for DIY forms. When someone enters a phone number, I am unable to tell whether it's a voice number of mobile number. Having this capability will enahcne the accuracy of the donor database.

    I have confirmed Etapestry makes the required phone field "Voice." This is both not what most people use these days (everyone is moving to cell phones), but it makes sense to create a phone field that links correctly to personas contact information for both the Voice (landline) and mobile fields. There is currently no way to make the form automatically link to the mobile phone field listed under personas.
