For a DIY form that I created using the Contact Information template that will be used to collect outgoing donation requests the pop up you receive after hitting submit says "Registration Successful" as the title. Registrations have nothing to do with this form. Although you can easily customize the subtext it doesn't make sense and is out of context to have an uncustomizable header.
