Limit the number of required fields in DIY form newsletters and events

Signing up for a newsletter or an event should be made easy for our supporters. However, current DIY forms have too many required fields. The fields 'Country', 'State' (only suitable for the US), 'Adress' and 'telephone number' could also be made optional in order to make simple forms that will not drive away our opportunities.

    We are now (2018) receiving complaints from donors about having to provide phone and e-mail information.  We need this change for donation forms also.
