DIY forms options for Business, Family and Individual contacts

With the name name settings, the DIY forms need to be updated to either allow the user to select the name format (Business, Family, Individual) or to have an option to set a form to be automatically setup for a name format.  It is silly to ask a business to enter a last name or to have to enter dummy information.

    Yes! Yes! Yes!

    I just discovered that DIY forms aren't appropriate for businesses and I've been using them for underwriters for our annual event! Please make this change ASAP. Takes a lot of effort to convert these entries once posted - and I'm sure it's very confusing for my donors!

    We are planning to make up an update to DIY pages this year to allow the option for donors to select Individual or Business. I will update this idea once the feature is released.

    Ditto what everyone else says. We need this to be able to be done. Our income almost doubled when they can do it online, but it is a mess on the back end.

    I completely agree! It is VITAL for a business that is giving to be able to do so under their business name!

    YES!!! If the database has individual, business, and family accounts, how can you collect donations that match to existing accounts if you only allow the entry of individual contact information!!!!

    Totally agree! I love DIY forms mainly for the efficiency they help create for our organization, but we are having to spend time editing accounts if they are businesses but are entered as individuals through the DIY forms into eTap. PLEASE update this feature ASAP!!

    My organization desperately needs this. We do tons of surveys that use the business name, and cannot use the eTapestry DIY survey forms until this is done.

    I just submitted this same request not realizing it already was posted... PLEASE work on this as without this functionality your forms are basically creating duplicate accounts, inaccurate donor records, and/or additional data entry for eTapestry users.
